We accept cash, apple pay, HSA, debit and all major credit cards.
*Note about Medicare: Medicare DOES NOT pay for the initial examination. There is a $150 fee during the 1st initial visit.
*Note about United Healthcare Community Plan- there are no chiropractic benefits for adults over 21 for this policy. All UHC HMO policies (including UHC Community Plan) require a referral from their PCP. The PCP must upload the referral through the UnitedHealthcare Provider Portal before the patient is seen.
We also accept insurance that we are OUT of NETWORK with if the policy has out-of-network benefits. You can see if you have out-of-network benefits by looking for "OON" on your insurance card.
